Behavioral Health Connection

Behavioral Health Connection

When patients need more care than the primary medical care provider can offer within their clinic, C-PAC care coordinators are available for complex care referrals. Because C-PAC covers all payer sources, including uninsured, behavioral health referrals are made  only to those professionals who accept the patient’s insurance.  In addition, all attempts are made to provide only referrals to providers who have open availability to accept a new client, want to work with primary care, have expertise in the presenting symptoms, and are geographically accessible for the family.  This process not only helps the patients and families find the right behavioral healthcare, but it strengthens integration of care between primary medical care providers and behavioral health specialists.
